Medicaid Waivers
MEDICAID WAIVER PROGRAMS
1.Developmental Disability Services (DD)
We provide services to individuals with intellectual disabilities and/or closely related developmental disabilities under two Medicaid Home and Community Based Waivers known as the Comprehensive Supports Waiver Program (COMP) and the New Options Waiver Program (NOW).
- Community Living Support (CLS) Skilled Nursing
- Community Living Support (CLS) Daily and 15-Munite Increment Services
- Respite Care In-Home Services
2.Community Care Services Program (CCSP)
Community Care Services Program (CCSP) assists older and/or functionally disabled consumers, their families, and caregivers in achieving safe, self-reliant lives. Participants who serve under this waiver must meet the same medical, functional and financial eligibility criteria as for placement in a nursing facility under Medicaid and must be approved by a physician.

3. Source Options Using Resources in a Community Environment (SOURCE)
SOURCE is a State Plan enhanced primary care management program that serves frail elderly and disabled beneficiaries to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services.
4.Georgia Pediatric Program (GAPP)
Under The Georgia Pediatric Program (GAPP) Skilled Nursing, we provide services to medically fragile children with multiple system diagnosis. Services are provided in their homes and communities and in a “medical” daycare setting as an alternative to placing children in a nursing care facility.
5.Structured Family Caregivers (SFC) – SOURCE
At Metnurse Health Services, Inc., we provide older adults with intellectual and developmental disabilities and their aging family caregivers an option to continually receive quality care at home. In-home, personalized care will be given to seniors with disabilities and complex health needs who are under the SOURCE waiver and living with a paid caregiver.
Services include:
- Personal Care
- Homemaker
- Medication Oversight
- Transportation Assistance
- Assistance with Bill Paying and/or Correspondence
- And More
6.Structured Family Caregivers (SFC) – CCSP
Individuals under the CCSP Waiver Program are eligible to receive SFC services from Metnurse Health Services, Inc. We offer financial and support services to the families of a senior or disabled individual who wishes to receive the care they need at home.
To become a paid caregiver of a family member at home, one must meet the following requirements:
- Lives with the patient as the primary caregiver
- Biologically related as a daughter, son, grandson, etc.
- Or related through marriage as son-in-law or daughter-in-law
- Does not have any other work other than being the primary caregiver
- Does not run an online business
- The person you take care of is enrolled under the Georgia Medicaid waiver programs, CCSP and SOURCE.
Services include assistance with:
- Eating
- Bathing
- Meal Planning and Preparation
- Dressing
- Moving Around the House
- Transferring
- Getting In and Out of Bed
- Positioning
- And More
7.Independent Care Waiver Program (ICWP)
We offer care services to adults with physical disabilities and traumatic brain injuries who want to live in their own homes instead of hospitals or nursing homes. Adults who can qualify for this service must be eligible Medicaid members with severe physical disabilities and are 21 to 64 years old.
